ROHS承诺书

为适应市场和客户的环保要求，加强对环保物料的品质控制和过程控制，规范供货行为，保证向苏州市龙基科技有限公司供应的化学原材料等均符合本承诺书中的要求，任何未达到承诺书中要求而造成的损失均由供应商负全部责任并进行相应的赔偿，特此需供应商签定此承诺书。

1.总则

自承诺书签定日开始所提供给苏州市龙基科技有限公司的产品，保证满足以下要求。否则同意苏州市龙基科技有限公司作拒收处理，并按以下条款3违反承诺事宜中条款作相应处理。

我方保证产品中环保控制指标为：

? 镉含量小于70ppm；

? 铅、汞、六价铬、多溴联苯（PBB）、多溴二苯醚（PBDE）含量小于800ppm；

2.具体条款

供应的所有物料均满足ROHS要求并保证产品满足REACH要求。供方需提交：有效期内的RoHS第三方(SGS或CTI)测试报告（中、英文版）。 所有检测报告有效期为一年，若我方临时需要供方提供检测报告，供方应给予积极配合。

3.违反承诺事宜：

凡我方提供的所有物料，若不符合本承诺书要求时，同意做如下处理：

3.1在苏州市龙基科技有限公司进料检验过程中发现不符合本承诺书要求时，同意退货并同意处以当批货物总值2倍的罚款。

3.2在苏州市龙基科技有限公司生产过程中发现不符合本承诺书要求时，同意扣除由此产生的工时、材料连带等费用及当批货物总值2倍的罚款。

3.3在苏州市龙基科技有限公司客户端发现环保不符合本承诺书要求时，同意扣除此批产品的2倍货款及连带质量损失费用，并对同意以1000-100000元RMB罚款。

4.此承诺书自签署之日起生效。

RoHS/REACH供应商及加工商承诺书 Green product parts supply warranty

为确保东莞市XXXX有限公司制造的产品符合无毒或低毒环保要求，及符合各国环境保护之法律法规，请上述供应商针对附件一《电子电气产品中特定有害物质的使用限制的指令确认表》所供应我司之所有物料及元器件，提出保证，以明确双方权利和义务。

To ensure the products we Ceros(DongGuan)Plastic technology Co.,Ltd manufacture meet the requirement of RoHS and related

international environment protective law, the supplier should provide us warranty for all parts contained in the attached Green Product confirmation table to verify the responsibility of both two sides. 保证之内容如下/warranty:

A、原材料不使用重金屬元素镉、铅、汞、六价铬、以及多溴联苯醚、多溴联苯等阻燃剂等有毒成份，详细清单以附件二所列《绿色产品材料限用物质含量标准》之各项成分为主。

B、所有料件均通过SGS等第三方公正单位化验合格的产品，且具有书面之第三方的化验合格证明书要求化验的项目及相关合格规范亦请依照附件二办理。

All parts should be verified by the third part such as SGS, and approved by written consent of the third part all testing items and related specifications should be in accordance with attached file 2.

C、如有发生供应商所交货料件不符合本保证书内容（该料件详列于附件一"绿色产品禁限用物质调查确认表"），其所发生的有形及无形损失概由供应商负担全部责任。

If the information submitted by suppliers fail to meet the warranty,（see details in the attached file 1"The Investigating Confirmation of the Prohibited Substance in the Green Products".）the supplier should be responsible for either invisible or visible loss.

REACH承诺书/Certificate of Compliance

We declare that all our products for Ceros(DongGuan)Plastic technology Co.,Ltd with the provision as follows: REACH Regulation No. EN62115、EN71 、ASTM F963

我们郑重声明：我公司提供给烁石塑胶科技有限公司的所有产品符合以下条款要求： REACH规定No. EN62115、EN71 、ASTM F963

All of our products are in conformity with the provisions of the Regulation EN62115、EN71 、ASTM F963 of the European Parliament Council of 18 December 2006 concerning the Registration Evaluation, Authorization and Restriction of Chemicals.

Furthermore the products do not contain the Substances of Very High Concern Shown below and listed in Annex l of the

regulation, as measured by weight in homogeneous materials.

As the Supplier to Ceros(DongGuan)Plastic technology Co.,Ltd, we are willing to provide all necessary documentation or information, to prove our compliance with the above provisions herein. It is our responsibility to proactively inform Ceros(DongGuan)Plastic technology Co.,Ltd, we identify that there is a potential risk of breaching the restricted and banned substance regulations in our products.

我们提供的所有产品均符盒欧洲议会理事会在20xx年12月18日通过的EC1907/2006《化学物质注册、评估、授权及限制》的相关规定。

而且，产品中不含有禁限用的高度关注物质可以以均质材料的形式予以称重。

作为东莞市XXX有限公司的供应商，我们乐于提供所有必要的方件或信，以证明我们的产品符合上述条款。若我们发现产品有违反禁限用物质规定情况而存在潜在风险，则有责任在第一时间通知东莞市烁石塑胶科技有限公司。 如有争议，双方同意以东莞市中级人民法院为管辖法院。

Both sides agree to bring into the Shenzhen secondary people court if any disagreement accrues.
