使用时，请根据自己高中的实际教学情况，确定每一个学期开设的课程。

几个小建议：

统一换算成百分制，这样便于国外院校理解

如果不嫌麻烦，可以将中文成绩单和英文成绩单分开做

该学习没有的课程，请用“/”表示

国外要求平均分至少在80分以上，而不是我们国内说的及格60分就行

成绩单要加盖学校公章或教务处的章，有些学校会有成绩单专用章

成绩单如果不做工作，原则上需要装入信封，密封，封口处要盖章。
