3、报告无主检、审核人、批准人签名无效。

（The report is invalid without the signatures of the persons for Chief tester, verification and approval.）

4、报告涂改无效。

（Test report is invalid if altered.）

5、试验报告仅对来样负责。

（Responsibilites of the test report are undertaken for the delivered samples only.）

6、对检测报告若有异议，应在报告发出后15个工作日内提出。

（Different opinions about the report should be reported to the test department within 15 days from the date of receiving the report.）

化学锚栓抗拨试验报告

试验内容：在混凝土中，埋植M12化学锚栓，采用ZY型锚杆拉力计进行锚栓

抗拨试验。

化学锚栓：M12（5.8级）邯郸市旭金德化学标准件有限公司

试 验 结 果

注：抽样数量和位置由监理指定。

结论：根据《混凝土结构后锚固技术规程》JGJ145-2004进行试验，本次现场检测的M12（5.8级）化学锚栓固承载力合格。
