1.       個人資料 / Personal Information

2.       研修計畫 / Plan of Study

3.       住宿 / Dormitory

4.       健康情形 / Personal Health Statement

如有疾病或其他，煩請敘明之/ Please state your health conditions or diseases if any.

5.    檢附資料（請查對應備文件是否已備齊，複印資料的大小應以A4紙為限）

C hecklist for materials to be submitted. The size of paper used for photocopies should be A4-sized.

(1)       本申請表

(2)       研修計畫書Plan of Study

(3)       學校出具的成績單正本乙份(包含總平均與排名) Official transcripts

(4)       校方開立之在學證明函正本乙份

(5)       推薦信函一封One letter of recommendation (original)

(6)       學生證影本一份Photocopy of Student ID, and ID Card

(7)       大陸地區居民身份證影本一份Photocopy of ID Card

(8)       最近三個月內二吋正面半身彩色白底照片一張One2 x 2 head shot photographs

(9)       已填妥大陸地區人民入出臺灣地區申請書（請以繁體字填寫各項欄位資料，貼上規定格式之相片與身分證正反面影本；申請人並須親自簽名）

(10)   健康檢查表（請詳閱健康檢查表內相關說明，並須完成表上所有檢查項目並加蓋檢查醫院證明章，可另附證佐資料）Health certificate

(11)   其他證明文件（非必要）Other supporting documents. (optional)

7.      推薦學校確認 / Agreement by Home Institution
